EMPLOYEE SELF SERVICE

Canada Post Corp. has initiated a Employee Self Service Program. Starting February 8, 2004 you can review and update your benefits, or update your personal data, by visiting a secure website, accessible from any computer with internet service. Your benefits at Canada Post are a key part of your overall compensation program and Canada Post prides itself in offering a competitive benefit package that you can now access more easily.

ESS has been designed so that you can find information and make necessary changes directly to your employee file at any time, quickly, easily and conveniently. You will receive a Package containing a Brochure that tells you more about ESS and how to access your personal data when you need to review or change information. Once you get your package with your User ID, and Password, then you can go to the website and check it out.

AIL LIFE INSURANCE CORP.

There is also an optional free Family Vision care discount program with 40% off from Sears Optical & Pearle Vision Optical which is not automatic like the accident insurance. If you filled out and mailed back the yellow card then a representative would contact you and issue you the family discount card.

Only by returning the card will you be contacted but the Association is the Master Policy Holder, so all the members are covered for the accident insurance regardless of returning the card. Members working part time hours have these benefits apply to them as well.

If you would like more information you can call directly, American Income Life at: toll free 1-866-236-4699 or e-mail them at questions@altig.com or telephone at 403-258/-2027 or Fax at 403-252-7772.

WHEN AND WHO YOU SHOULD CALL ABOUT THE DISABILITY INSURANCE

The Disability Insurance (DI) plan, administered by Canada Post, is provided through the Sun Life AssuranceCompany of Canada. The plan provides you with 70% of your adjusted annual salary if you are disabled for a long prolonged period of time or if you become totally permanently disabled. There is an elimination period (waiting period) of thirteen (13) weeks or when accumulated sick leave credits are exhausted, whichever is greater.

In the event that you have insufficient sick leave credits at the time of your extended illness or injury, interim Employment Insurance (El) benefits may be available to you until your 13-week elimination period has expired.

All regular (term employees not eligible) full-time and part-time employees working a minimum of 131/3 hours (survey hours only) are automatically enrolled into the plan. There is one exception - employees who opted out of the plan when it was administered by Treasury Board. These employees are not automatically enrolled into the plan when they meet the eligibility criteria described above. They may apply for coverage as a late applicant but must provide Sun Life with a completed Health Questionnaire demonstrating evidence of good health. In these cases, coverage would commence the first day of the month following the date the Benefits office receives the application and Health Questionnaire (subject to Sun Life's approval of the health Questionnaire). When you are sick for short periods of time, you are able to draw on your accumulated sick leave credits to protect your income. However, the DI plan is intended to provide you with income if you are totally disabled and unable to work for a prolonged period of time. The following sequence of events should occur to ensure that you do not suffer a financial hardship during this already trying time.

Once you are in a situation where you are unable to attend work due to a lengthy illness or injury, you need to determine how many sick leave credits you have accumulated. This information is mailed directly to you on a quarterly basis from the Payroll Deportment (Quarterly Leave Report). You should contact the CPAA Compensation Support Center (1 -877-727-2722) and your union representative, whenever you are aware that your absence from work due to illness will be long term. The Compensation Support Center will advise your Benefits officer who, in turn will contact you for relevant details and follow up. As appropriate, the Benefits department will send you a Disability Insurance Claim Kit, and subsequently answer any questions you might have on how to complete the forms. There is also a form in the kit, a new authorization form for union representation and the disclosure of medical information. It Is of the utmost importance that you fil! out this form. This form allows the employee to authorize a union representative to represent him / her while receiving disability insurance. Once this form is signed, this gives the union representative the right to have discussions with Sun Life prior to any change:

  • In the status of rehabilitation
  • When you are expected to return to work in the near future
  • Before stopping your disability payments
  • If your disability is declared total and permanent

As Sun Life requires 4 -8 weeks to review and adjudicate your claim, completion of this kit should be done as quickly as possible. If Sun Life approves your application for disability benefits, you will receive 70% of your adjusted annual salary. If you are currently on extended absence due to illness and have not yet spoken to anyone in the Benefits Department, you should immediately call the Compensation Support Center at 1-877-727-2722 and give the details of your absence. Your Benefits advisor will be in contact with you as soon as possible.
